Is early remdesivir treatment associated with lower 1-year all-cause graft loss among kidney transplant (KT) recipients with symptomatic COVID-19? This cohort study emulated a target trial using retrospective data from 432 KT recipients, of whom 41% received early remdesivir within 7 days of COVID-19 diagnosis and 59% received no remdesivir. Early remdesivir vs no remdesivir was associated with a statistically significant lower risk of 1-year all-cause graft loss. The findings support timely consideration of remdesivir for eligible KT recipients and suggest that longer-term graft outcomes should be included when evaluating COVID-19 antiviral strategies in this population.
