Does cardiovascular-kidney-metabolic (CKM) stage modify the effects of the nonsteroidal mineralocorticoid receptor antagonist finerenone? In this post hoc analysis of 2 phase 3 clinical trials that included 12 990 participants, 3864 (30%) were classified as CKM stage 2, 3275 stage 3 (25%), and 5851 stage 4 (45%). Finerenone consistently reduced cardiovascular and kidney events, irrespective of baseline CKM stage; finerenone treatment was additionally associated with a higher likelihood of CKM syndrome regression, and a lower likelihood of progression, over time.In this study, finerenone reduced risks of cardiovascular and kidney events across CKM stages and had favorable effects on CKM syndrome evolution.
